Consent to Medical and Release of Liability

I hereby permit my child to participate in the Clinics/Instructional sessions offered by Stacy Tamborra and Champions Fast Pitch Academy, Inc. .  By the execution of this release I acknowledge and agree that all requirements, directions, supervision and standards set by the directors of this program shall be established for his/her benefit.  The person enrolling for Champions Fast Pitch Academy Clinics, Lessons, or Instructional Sessions, his/her parents or guardians assume all risk of loss of property or injury to the person, including injuries associated with softball activities, speed, and/or strength camps.  I agree that there are inherent risks associated to softball participation and therefore agree to hold Champions Fast Pitch Academy and its employees harmless and specifically agree not to make any claim against Champions Fast Pitch for any of these injuries which may be considered normal risk associated with participation in softball activity.

I hereby voluntarily assume all risk of injury, of any form, to my child, which may arise out of his/her participation in this program, hereby intending to release Stacy Tamborra and personnel associated with this program from liability that may result from his/her participation.  In addition, I hereby give my permission for emergency medical treatment in the event I cannot be reached.

2007-2008 Lesson Fees

Senior Instructor

1/2 hour Private Lesson.........$45

1 hour Private Lesson..........$70/hr

Single Group Lesson............$45/hr

*Monthly Group Lessons........$155/month

Team Training.......$80-$115/hour

*Monthly Group lessons are based on a 50 week year and it is a flat rate that enables a consistent payment because we know that some months might have 3 weeks and some have 5. This is the best way that we can assure that consistent payment.

Junior Instructor

1/2 hour Private Lesson.........$35

1 hour Private Lesson..........$60/hr

Single Group Lesson..........$35/hr

Monthly Group Lessons.........$115/month

Junior Instructors are Champions Elite Athletes who are Juniors or Seniors in High School who have been trained on how teach teach beginning pitching/hitting/fielding progressions.  Once athletes have advanced enough the athlete will be promoted to a Senior Instructor.

2007-08 Cancellation & Payment Policy

1.  All payments for private lessons and single group lessons are due at the time of the lesson.  Fail to pay for each lesson will result in a $25 late fee and client will not be able to schedule an additional lesson until the invoice has been paid.

2.  Monthly group lesson payments are due by the first lesson of the month receiving lessons or by the 10th of the month receiving lessons, which ever payment opportunity comes first.

3.  Late group lesson payments will result in a $25 late fee.

4.  If a lesson is missed due to illness, team practice, games, etc. it is the client's responsibility to contact Champions and schedule a make-up lesson, there will be no refund provided for the missed lesson. 

5.  The client must contact Champions at least 8 hours prior to the lesson to be provided a make-up.

6.  For each missed lesson the client has 30 days to make-up the lesson, after which no make-up will be provided.

7.  The instructor is required to offer 1 make-up day per month for all missed lessons.

8.  If the lesson is cancelled due to weather or instructor conflicts, the instructor will contact the client for a make-up session.
